You are holding the lighter upside down while refueling. If you don’t turn the lighter upside down, there will be air inside the butane flask. You’ll need to get all the air out and try refilling.
Flame height is adjusted accordingly.
You are not using cheap or improper fuel.
There is no dirt or debris in the lighter. Try blowing hard on the top of an unlit lighter.
